Diabetes Clinic-individual consultation with your Doctor, Dietitian, Diabetes Educator and Podiatrist on a rotation basis to review care using a team approach. To book into this clinic please contact your Doctor or call the Splatt St Medical Clinic on 50331711. This clinic is held every fortnight and will take approximately 2 hours. Services provided from 73 Beveridge St., Swan Hill Diabetes Education- individual consultation regarding monitoring of blood glucose levels, insulin use, education on the disease and its affects, for appointments phone 50339337. Diabetes-The Facts: two session education program. Provided by a Dietitian and a Diabetes Educator on the first and second Tuesday of every month, alternating between an evening session (7pm-9pm) and a daytime session (10am-12noon). This program is free to participants and aims at providing up to date information for those newly diagnosed with either Type 2 Diabetes or Pre-Diabetes, and for those who haven't recently recieved any education.
